Please take the time to fill in this questionnaire and answer to the best of your ability. In order to obtain your certification you must complete all 5 questionnaires (1 for each training day). All training modules must be completed and all subsequent questionnaires answered with a pass rate of 50% or higher in order to obtain your certification.
